By signing my name, or typing it below, I hereby waive my rights as indicated by the date entered on the end of this form, to sue InFaith or their employees offering programs through their ministry and mission. I release my rights to sue for any personal, physical, emotional, or property damages to me or to my children during any InFaith, and InFaith: Heart Ink, sponsored activities while on the designated property of Calvary/El Calvario Church located in Whittier, CA. I agree that I of my own accord have agreed to participate and have given permission for my children to participate in these activities. I understand that the directors and volunteers have done their due diligence to provide a safe learning environment in compliance with CA AB506 while working with children, and of my own free will, I have released my child into their care. All Heart Ink volunteers are considered mandated reporters for child protection in compliance with CA AB506 laws.
Heart Ink Classes: I understand that I am encouraged to join my Pre-K to 1st Grade student (on TUESDAYS) to assist them in activities and feel comfortable in the classroom. For all other ages (Summer Art camps, Thursdays and Fridays) I understand that I can wait outside the classroom to monitor things if I feel uncomfortable leaving my child, or I can drop off my child and return for pick-up.
ALL CLASSES: I understand that if I choose to stay in the classroom or outside, I am not to interfere with the teaching instruction in a disruptive manner or contradict the teachers as they work with my child to develop their artistic skills. I agree to support the lead teachers and volunteers for the benefit of my child's growth, and if there is an area of concern, respectfully address it with the director. In no way, will I approach dealing with any conflicts with a volunteer, teacher, other parents, or children in an aggressive or abusive manner.
I will also follow appropriate restroom protocols for child safety. I will never enter the restroom facility with any other minor, except my own child. (Minors includes all students under the age of 18).
I agree to: 1) Drop off and pick up my child at the end of each class promptly and come to the front of the classroom door for pick up and communicate by a phone call to the director, if I am unable to do so for some emergency reason, and 2) I agree to check in/pick up my child using the online security system with smart phone access, or through a paper sign in/out forms as necessary.
I agree to create a safe and encouraging environment as a supportive parent by agreeing to the following and enforcing the following with my children: 1) I, and my child, will give positive encouragements to each other and to other students. I will not criticize or demean anyone else or their artistic work 2) I, and my child, will not use profane language in order to respect the culture of other families while in classes 3) I, and my child, will by no means ever bring onto the Calvary/El Calvario property any: drugs (including vaping and cigarettes), alcohol, weapons, or any item that would be used to harm another person or the property. This will result in immediate removal from all activities.
It is our intention to keep your children safe while during class and on campus, and this includes Covid-19 precautions. We will continue to respect the CDC regulations as given for classroom (school) pre-cautions, as they change and adjust with situations which can include wearing masks or not, sanitizing surfaces, and keeping social distances based on family needs.
By signing below, you also agree to keeping other families and children safe by the following: 1) Answering any pre-screening questions or temperature checks as necessary 2) Notifying the director if you or your child has been exposed to Covid-19 while also attending Heart Ink classes so we can notify the class of an exposure while keeping it confidential 3) Quarantining at home and not attending Heart Ink Classes if you have been exposed or tested positive, until you have a negative test result 4) Agreeing that you, your children, and others you bring with you, attend classes at your own risk, and you agree to release InFaith and those serving in the program of InFaith: Heart Ink (including directors to volunteers) from any and all liabilities associated with Covid-19 risk factors during your participation and attendance.
Heart Ink Classes (only): I understand that the stories we will be sharing come from the Bible.
Art Summer Classes (only): These classes will have a spiritual theme, but no Bible story.
Friday Fun Days: These classes are nonsectarian and do not have any faith or religious content in the teaching.
I, and my child(ren) are free to respectfully process our beliefs as we agree, disagree, or arenāt sure what we believe. Should I have any concerns in this area, I will respectfully talk to the director or teachers. (Please don't hesitate to ask us questions should you have them!)
In the case of an emergency, where the parent or guardian is absent, no longer conscious, or unable to respond, I, the undersigned, give my consent for my registered child identified below to be transported by InFaith and/or InFaith: Heart Ink personnel and/or volunteers (hereinafter āInFaith: Heart Inkā) to appropriate facilities (such as a hospital) or to the listed family emergency contacts. I agree that I will hold InFaith: Heart Ink, its officers, agents, employees, assigns or anyone acting on its behalf, harmless for injury occurring to my child in the course of such travel. This travel is not limited to designated property of Calvary/El Calvario Church located in Whittier, CA. In addition, I authorize InFaith: Heart Ink to obtain, through a physician or hospital of its own choice, any emergency medical care that may become reasonably necessary for my child in the course of such travel and agree to accept the cost of the transportation and/or treatment by medical personnel or facility. I do hereby give express permission for my registered child to ride in a vehicle driven by an approved and licensed InFaith: Heart Ink employee or volunteer over the age of 21 only under said emergency conditions for the duration of my childās participation in InFaith: Heart Ink activities.
I hereby agree to the Liability release statements on this form, and the parent agreement statements for the code of conduct during my involvement with all activities under InFaith: Heart Ink.
