Add a child you are registering. Make sure to include medical notes and special learning needs for each child, but leave it blank if it is not applicable. And please describe how we can provide care and take pre-caution for your child while in the class.

I hereby give my permission for any and all photographs or videos taken of me, or my children, to be used in InFaith, or InFaith: Heart Ink publications, displays, brochures, websites, or social media posts for marketing and communication purposes. I warrant that I am over the age of 18 and I am free to enter into this agreement.

This typed name is therefore equal to an original signature.

NOTE: In our FREE Heart Ink classes of our goals is to specifically reach families who would not normally have access the arts due to economic or financial limitations, have need for emotional and mental health support systems, need affordable after-school child programs, and those who desire to explore spiritual questions (Heart Ink Classes and Summer Art Camps only). Priority can be given to those with greater need according to these goals.

FINANCIAL PAYMENTS: I understand that after my application has been completed, and approved. I will receive a link for making payment for Heart Ink classes. The cost is for this Spring 2025 is $5 per class per child which will be $60 for each student for the 12 week term. If we can we would like to keep these classes free especially for low income families, but are in need to charge a small fee at this time, hopefully not for long. If you are unable to make this payment, please let us know! We are working to set up scholarship funds, so that we keep these classes available for those who are most needing creative expression for mental health and equal opportunities where economic status is not a factor. We encourage families that have the ability to give more, to consider giving a larger donation to help us cover rent, supply costs, teaching hours, or "pay if forward" for another family. Your gifts help keep our teaching free for the community!
